The form is available in three different languages - English, Spanish and Creole. Simply download the Application for Divorce or Annulment Record form and fill in the details. Remember to include all the important information such as the name of the husband and wife and the city or county where the divorce was obtained.

People who request for the record should remember to indicate the complete name and information of the divorced parties especially the maiden name of the wife. Should one be unsure of the exact details, it is crucial that he or she remember the husband's name as the Bureau indexes the divorce records under the husband's name.
